If you have received an invitation to the adidas Elite Soccer Program 2009, Congratulations, you may select to register online. Your User Name and Password have been printed on your invitation. Please select the 'Register Online' link above (or Login on the bottom of any page), type in your User Name and Password. You will be given specific access to online forms that you can fill out. These are the forms required for you to attend the adidas Elite Soccer Program.

We have highlighted below which forms are required to be filled out online and which forms are required to be downloaded, filled out and mailed to the address below. These forms require signatures by either your Doctor or Parents/Guardians.
